A company is looking for a Behavioral Health Care Manager (BHCM) - Hybrid.Key Responsibilities:Conduct initial clinical member biopsychosocial intake and administer behavioral health assessmentsCoordinate care and act as a liaison between specialists and the internal care teamLead weekly interdisciplinary team meetings and document member progress in a HIPAA-compliant systemRequired Qualifications, Training, and Education:LCSW licensure and licensed to practice in MarylandBachelor's Degree in nursing, social work, or related health and human services disciplinePreferred experience as a Certified Case Manager, Community Health Worker, or Peer Support SpecialistAt least one year of clinical care management experience, preferably with cancer patientsBilingual in English and Spanish
