DescriptionPHP Alabama is searching for our next Case Manager to coordinate the habilitation process for an amazing group of individuals with intellectual and developmental disabilities. They will assure that each individual's quality of care, services, and training is provided in accordance with Federal and State Standards. Salary commensurate with experience.Responsibilities:Ensure that individuals are assessed by Professionals/Consultants who report the correct level of functioning for the initial ND annual habilitation plan. Trains all staff members on the appropriate implementation of training programs.Facilitates the exchange of program information regarding strategies, techniques and progress among and between disciplines and shifts.Facilitates all or part of meetings regarding the individual to include but not limited to interdisciplinary team, habilitation plan, ect. Ensures that Federal and State regulations are followed by all staff assigned to them and that house and equipment meet or exceed all federal and state regulatory requirements.Provides direction to staff to ensure overall program consistency and compliance with applicable State and Federal standards. RequirementsA Bachelor's degree in Human Services or a similar related field from an approved, regionally accredited college or university required.One (1) year of experience working with intellectually and developmentally disabled individuals required.A minimum of one (1) year, previous supervisory experience.Valid AL Driver's License with an acceptable driving record for the past three years.Pass a level II background including TB test & drug screening.Must have excellent writing and communication skills.
