Description:

Job Location Montgomery, Alabama Minimum Education High School Diploma or GED Equivalent Work Experience Appropriate to the position duties described. Job Duties * Schedule, confirm, and directly transport clients to ensure they arrive on time for all appointments by planning and following the most efficient transportation route

* Ensure all clients have an up-to-date transportation waiver on file

* Collaborate with medical and social services work staff to determine urgency of transit needs

* Determine most effective transportation resource and provide linkage for transportation resources, including public transit (bus passes), personal vehicle (gas cards), Medicaid non-emergency transportation

* Report any vehicle mechanical problems to supervisor and keep van exterior and interior clean for passengers

* Comply with traffic regulations in order to operate vehicle in a safe and courteous manner

* Accommodate clients with disabilities by loading and unloading wheelchairs, crutches, braces and other medically necessary items

* Demonstrate and relay a skilled knowledge of HIV / AIDS issues and their effects on individuals, families, and special populations

* Maintain required client records, document services provided, and complete other required paperwork in a timely manner

* Maintain compliance with HIPAA and state guidelines as well as MAO policy and procedure

* Participate in continuous quality improvement and outcome evaluation activities

* Provide individual and group educational skill building opportunities to foster adherence to medications, identify strategies to improve adherence to health routines, communication with providers and additional issues to increase engagement in care and adherence to treatment

* Conducting informal assessments of clients need for primary care/treatment and/or supportive services, early intervention/diagnosis information, peer based counseling, and general assistance in accessing appropriate services

* As with all other positions, during emergencies, the Transportation Resource Specialist is expected to fill in at any position within the agency for which he / she is capable of performing

* Perform any other department / agency related duties or special projects as directed by supervisor
